Destination : A Real Home is a unique, one-of-its-kind shelter home cum rehabilitation center for destitute, homeless, abandoned adults with mental health challenges and also special needs adolescents who are transitioning into adulthood. It provides post-treatment care and rehabilitation for individuals discharged from mental hospitals and asylums, addressing the societal reluctance to reintegrate them. The center also accommodates special children from LRHMH's CCI who turn 18, as no other suitable facilities are available for their continued care. Supported by the Department of Social Justice and Empowerment, Government of Assam, along with individual and organizational donors, the project offers holistic rehabilitation through counseling, vocational training, music, yoga, medical and psychiatric support, and other need-based interventions. The aim is to help beneficiaries develop self-sufficiency and gradually reintegrate into society. Currently, the center supports 50 residents, focusing on structured care, emotional healing, and skill development to ensure dignity and purpose in their lives.
